Have taken a look at this website Wufuu that enables very quick set up of web forms. These are the sort of "Join my mailing list" "Contact me" sort of things that you see on websites.
Now while I use Mailchimp for my mailing list, and they have a very robust form that enables people to subscribe to my mailing list - complete with ways to opt out so you do not violate anti-spam rules, I think the surveys on Wufuu might be handy.
